The Day of Your Appointment

Please arrive 10-15 minutes before your scheduled appointment to register. By having the following items ready we can more efficiently get you checked in:

Checklist for the day of your appointment:

• Photo ID
• Insurance Card(s)
• Prescription from your referring doctor
• Referral form (if required by your HMO)
• All co-pays, co-insurance or deductible payments are due on the day of your appointment. We accept cash, checks and all major credit cards (except American Express).
